How to fill in your questionnaire - on paper
Your questionnaire will be scanned by a computer. To make sure we record your answers correctly, please follow the instructions below.
- Use black or blue ink
- Tick your answers within each box
- Print your answers, in English, within the write-in boxes – use capital letters, one per box
- Correct any mistakes by fully shading-in the incorrect box. Then continue writing in the box that follows.
- DO NOT draw a line through questions or pages. The computer may mistake this for an answer.
